Only include your middle initial if it appears on your card. If this is a company card, enter the company name as both a first name and last name.
How do I stop my card details being saved again?
Microsoft saved my credit card details and there was no option to refuse this. I checked previous questions and answers, and there was a thread that explained how to delete the saved card - which I did. However, when I went into the payment settings the box to automatically save card details was already unchecked.
