The extreme level of resonance produced along with sound on hitting the chest and abdominal area is known as hyper resonance. The changes in the resonance upon hitting can indicate a medical condition the patient suffering from. The hyper resonance if present on the left side of chest indicates the presence of pneumothorax.

Hyperresonance to percussion was the strongest predictor of COPD, with an LR of 9.5. Along with shortness of breath and pack-years, these three chest signs provided significant diagnostic information.

Percuss down the back until the normal hyperresonance of the lungs becomes dull over the diaphragm. Then simply have the patient breath in and out deeply while continuing to percuss. The sound should wax and wane.
